House League Registration

1. PARTICIPATION WAIVER: I hereby request and approve the player's registration to compete in the Hoop Culture House League. I certify that the participant is in good physical condition and fully capable of participating in strenuous basketball activities.

2. ASSUMPTION OF RISK: I understand that basketball involves physical contact and inherent risks of injury. I agree to assume all risks associated with participation, including but not limited to sprains, fractures, concussions, or other serious bodily injury.

3. LIABILITY RELEASE: I release, waive, and forever discharge Hoop Culture, DMC Basketball, Phil-Can Athletic Club, league organizers, referees, sponsors, and venue hosts from any and all liability, claims, or demands arising out of injury, loss, or damage incurred during league activities.

4. MEDICAL EMERGENCY: In the event of injury or medical emergency, I authorize league staff to obtain necessary medical treatment for the participant if a parent/guardian cannot be reached immediately.

1. RESPECT: All players must show respect to referees, coordinators, opponents, teammates, and spectators. Bullying, racial slurs, abusive language, or threatening behavior will result in immediate suspension.

2. ZERO TOLERANCE FOR VIOLENCE: Any form of physical violence, fighting, or intent to injure another player will lead to permanent expulsion from the league with no refund.

3. DRUGS & ALCOHOL: The possession or consumption of illegal substances or alcohol at any game venue is strictly prohibited.

I hereby grant Hoop Culture, ARBS Stats Platform, and their media crew permission to take photos, video recordings, and interview footage of my participation. I authorize the use and publishing of such media on stats cards, websites, and social media platforms for highlight reels, standings, and promotions.
